The Dallas County Health Department is encouraging residents to complete a survey for the Community Health Needs Assessment (CHNA).
The health department claims that the assessment aims at gathering valuable insights into the health needs and priorities of residents in Central Iowa. The topics in the survey will cover health care, chronic disease management, mental health, substance abuse, and more.
The assessment will share strategies with participants pertaining to public health. Dallas County is working alongside both Polk and Warren counties. The deadline to complete the survey is by May 24th. To take the survey, click here.
