Unit 2 parked black 2013 Dodge Ram pick up registered to David Lee Hutchison, 1815 5th St.,
Perry.
Unit 3 blue 2012 Toyota Sienna registered to Saul Garcia Hernande;:, 1615 5th Street, Perry.
Officers were called to the 1500 block of 5th Street on a report of a hit and run accident. The
run vehicle was later located. Total estimated damages $23000. Driver of unit one was cited
for striking an unattended vehicle.
Received report of a hit and run accident in the 1700 block of Lucinda. Case under
investigation.
November6:
Received a report of harassment in the 900 block of Willis Ave. Officers were unable to locate
the subject.
Received a report of theft in the 2500 block of Lucinda. Approximate losses of $350.00. Case
under investigation.
November 7
Received a report of a domestic situation in the 1000 block of Pattee St. Verbal dispute only.
Arrest: Cody Daniel Walker, age 21, 516 Clearview, Perry, was taken into custody and charged
with criminal mischief 4th degree, intimidation with a dangerous weapon, criminal mischief 5th
degree and two counts of simple assault.
Amado Loaiza, 2209 lucinda, Perry was cited for dog at Iorge 2″d offense.
NovemberS;
Received a report of a theft from vehicle in the 1300 block of 1st Ave. Later determined items
were misplaced. No theft.
Arrest; Fredy Espinoza, age 19, 7013’d St., Perry was taken into custody and charged with Theft
2″d Degree.
November 10:
Arrest: Erick Gabriel Salazar, age 21, 710 18th St., Perry was taken into custody and charged
with possession of a controlled substance 1″ offense. (Marijuana).

Unit 1 gray 2010 Chevy Impala registered to and driven by Sherry Lyn Ludwig, 210 S~ 2nd St.
Panora.
Unit 2- blue 2011 Nissan Rogue registered to Gerson Omar Carrillo, 1905 W. 2″d St., Perry
driven by sara Virginia Carrillo, 2225 McKinley St., Perry
Unit 1 was traveling northbound on l’t Ave Unit 2 was traveling westbound on Willis Ave. Unit
1 crossed in front of Unit 2 and was struck by Unit 2. Total estimated damages $5,000.
Accident:
Unit 1 white 2010 Chrysler Town and Country Van registered to Strom Cab LLC, 110 Mallard
Ave., Storm Lake, Iowa. Driver of unit 1 was Joann Abbie Spanier, 202 S Berlin Street, Schaller,
Iowa.
Unit 2 red 2010 Toyota Corolla registered to Fatima Ramos Hurtado, 518 211d St, Perry, driven by
Laisha Paola Hurtado, 516 2″d St, Perry.
Unit 1 w<~s westbound in the 1100 block of Willis Ave when she observed a traffic accident at lH
and Willis Ave blocking the roadway ahead of her. Unit 1 decided to improperly back up into
the intersection of 2nd Stand Willis where unit 2 was turning north onto znd Street and struck
unit 2. Total estimated damages $1000. Driver of unit 1 was cited with unsafe backing and
driver of unit 2 was cited with driving without a valid driver’s license.
November 11;
Arrest: Jill Ann Onnen, age 60, 523 Clearview Drive, Perry was taken into custody and charged
with violation of a no contact order.
Accident:
Unit 1 red 2007 GMC Acadia registered to and driven by Lacie Dlin Hemphill, 1418 61h St., Perry.
Unit 2 gray 2012 Ford Focus registered to and driven by Adeline Star Cook 528 Eli Circle, Ames,
Iowa.
Unit 2 was stopped on Pattee Street facing eastbound at the 71
h Street intersection. Unit 1 was
traveling north on 71h St. approaching the intersection with Pattee. Unit 1 failed to properly
navigate the intersection turning too sharply striking unit 2. Total estimated damage $2000.
Driver of unit 1 was cited for failure to provide proof of insurance, accident related.

November 13:
Officers responded to the 2900 block of Iowa on a report of a domestic dispute. Verbal dispute
between parties. Parties separated for the night.
November 14:
Arrest; Bihadndo Headman, age 21, 201 Woodland Ave., Des Moines was taken into custody
and charged with criminal mischief 3’d degree.
Officers responded to the 1700 block of 1’1 Ave on a report of a child struck by a vehicle. Child
did not have any marks. Unable to locate the driver.

investigation.
November 17:
Arrest: Daniel Robert Davis, age 22, 614 Willis Ave., Perry was taken into custody and charged
with child endangerment, serious injury.
Officers received a report of harassment in the 2600 block of Warford.
Received report of harassment In the 2000 block of 2″d Street.

Arrest: Joshua Abraham Perea, age 22, 1710 41h St., Perry was taken into custody and charged
with driving under suspension.
