A local public health agency is asking for feedback for what Dallas County residents feel are their top health priorities and time is running out to participate. 

The Dallas County Health Department is asking for feedback from community partners to help develop the 2023-2025 Community Health Improvement Plan after a recent public health assessment that was completed with Polk, Warren and Dallas counties. 

The purpose of this feedback is to find out what the top health priorities are in Dallas County and what action steps should be accomplished. 

The survey will be open through tomorrow and takes approximately 15 minutes to participate.  You can find the survey by going here
