D. Nicolaisen, age 30, 12842 Houston Ave, Perry, was arrested on a warrant for Failure To Appear.
May 8
Alejandro Perez Lazaro, age 29, 611 Maverick Lane, Madrid, was arrested for Operating While Under The Influence – 3rd Offense, Driving While License Denied Or Revoked. He was also cited for Open Container.
Glennis Smith, Rockwell City, was traveling on Hwy 141 near the Woodward exit, when her vehicle was struck by a car traveling in the lane next to her. Issac Lopez, Milan , Mo., was traveling on the inside lane. Mr. Lopez stated that wet conditions caused him to have trouble steering and he crossed the center line, making contact with Ms. Smith’s vehicle. No injuries were reported and damage was estimated at $400 for Ms. Smith and $200 for Mr. Lopez. Both vehicles were able to be driven from the scene.

Andrew D. McGregory, age 35, 290 NE Sunrise Dr, Waukee, was arrested on a warrant for Failure To Appear. The original charge was Driving While License Denied, Suspended, Cancelled or Revoked.
